Documents Rules Weather and missing child procedures Field Map and Parking Approved Application to host CHECK-IN: All teams must check in on Friday April 7th, 2016 any time between 6:00p.m. and 8:00 p.m. Out-of-town teams and teams playing Friday night may check in 1 hour prior to their first game at the Concession Stand at Midwest City Soccer Club. Check in should be completed at the following location: Midwest City Soccer Club Complex 1799 National Blvd Midwest City, OK 73110 Required CHECK-IN Documents:   Copy of official signed and stamped team Roster (Academy Teams may have unofficial rosters printed from GotSoccer) Current Medical Releases for all players- Medical Release Form Players Passes for U11 and Up Travel Documents for Out of State Teams   Dates: April 7-9th, 2017 Registration deadline March 24th Teams MUST be available to play ANY time, ANY DAY, during all three days of the tournament – no exceptions (out of town teams will not be scheduled on Friday evening).
