GUEST PLAYER FORM (if traveling from outside Indiana.  Indiana does not have a guest player form) ALL TEAMS WILL NEED TO CHECK-IN WITH ALL THE REQUIRED DOCUMENTATION ONE HOUR BEFORE THEIR FIRST SCHEDULED MATCH AT HEADQUARTERS.   HQ's WILL BE LOCATED AT THE TOP OF THE CONCESSION BUILDING.
