Notarized Medical Releases                                                                                                             2. State Cup Approved ID Cards                                                                            3. Referee will have Game Reports                                                                                                           **DON'T FORGET TO CHECK VOLUNTEER SCHEDULE.                                                                    Team Credentials:                                 Every Team will have (4) Team Credentials for registered coaches or managers.                                This means that Only (4) Adults will be allowed on the player's side line.                               Credentials are Mandatory – Must be Credential for team that is playing.                               Coach/Manager/ Trainer - Must have State Cup approved ID Card                                                          Before Start of each game - Hand to Referee:                                                                                          ID Cards                                                                                       End of Game:                                               GET "ID CARDS" BACK FROM REFEREE AND VERFIY SCORE                                               WINNING TEAM TURNS GAME REPORT INTO HEADQUARTERS
